Accommodation forms, landlord forms, verification forms... which is it?
These forms have different names, we usually refer to them as "RAFs" (Reasonable Accommodation Forms). These are forms that you may receive from your landlord to verify that the doctor who provided you your Housing ESA Letter is legitimate. Yes, our doctors are legitimate and we do have them complete many forms like this.
For Existing Patients:
You won't need to take our exam again, but you'll want to pre-sign your form (signing your name in the tenant/resident signature place) before sending us your form: support(At)emotionalpetsupport(Dot)com.
We will follow-up by sending you an invoice for it ($30) and we'll forward the form to the doctor for completion.
For Non-Existing Patients:
When taking the Emotional Pet Support online assessment, please be sure to select the accommodation form option ($30). After a doctor approves you, you can pre-sign the resident section of that form and email it over to us.
